For ALL patients IF you wear contact lenses*, please also complete the following (1 additional form to complete, if applicable):
*A contact lens evaluation is necessary if you would like a new or renewed contact lens prescription, even if you have done one in the past. If you decline a contact lens evaluation and your prescription is expired, you will be unable to order more contact lenses.
